You can sign up online by clicking on the "Sign Me Up Today" button to the left. This will send you back to the online registration page. Fill out the information on that page and click on the "Next" button. Check the "electronic signature certification" box. Once you click the "submit" button you will be an official donor. Then we will send you a letter with a donor card on it. Take the card off and keep it in your wallet.
Click on one of the buttons below if you do not want to register online to get a paper form you can use to sign up. Fill out the form and send it in by mail or by fax. You can use the form to
• sign up for the first time or
• change something on the form from before
The paper form lets you send in all the information and signature at the same time. It is faster if you sign the form before mailing to the registry. When we get the form with your signature, you will be an official donor. Then we will send you a letter with a donor card on it. Take the card off and keep it in your wallet.
